We present a case of a female, 7 pack-years smoker without chronic pulmonary symptoms who was diagnosed with recently appearing pulmonary nodules that suggested metastasis, for which studies were carried out to search for a primary tumor. A thyroid Ca was found coincidentally and was managed surgically. Due to the atypical radiological presentation, an open biopsy of one of the nodules was performed, which turned out to be a Histoplasmoma. A review of the radiological presentations of Histoplasma is carried out, this case being a rare presentation.
