Exaggerated decreased airway lumen during expiration, or central airway expiratory collapse, can be caused by weakness of the tracheal cartilage (tracheomalacia), bronchial cartilage (bronchomalacia), or both (tracheobronchomalacia-TBM). This weakness allows partial or near-total airway collapse during expiration, leading to symptoms such as dyspnea or chronic cough.
We present the case of a patient with significant neurological involvement and chronic cough. In this patient, a decrease in the diameter of the upper airway was found due to excessive dynamic collapse of the airway and tracheobronchomalacia. New diagnostic parameters and computed axial tomography techniques are discussed to achieve an adequate diagnosis with dynamic maneuvers.
