    We present a case of a cystic prevascular mediastinal mass that was going to undergo surgery, but because an old study 10 years earlier demonstrating its very slow growth, we decide on conservative management and periodic observation. We discussed the diagnostic possibilities and the most used imaging methods to reach a narrower differential diagnosis and make the best therapeutic decision.
