Rheumatologic diseases encompass autoimmune processes with diverse manifestations, including pulmonary disease. Sjögren's syndrome (SS) is characterized by lymphocytic infiltration mainly in exocrine glands, with interstitial lung disease being its most frequent and serious complication. The comprehensive approach to these patients should include radiological assessment to establish diagnostic suspicions in the context of an autoimmune entity. Based on radiological findings, a histopathological study should be undertaken to make an accurate diagnosis and thus initiate appropriate treatment. We present the case of a woman with a history of SS who develops exacerbation of respiratory symptoms, and after radiological and histopathological study of documented lung lesions, a diagnosis of Diffuse Lymphoid Follicular Hyperplasia is established.
