The following text presents a reflection on my experience as a professional in the field of Pneumology in Colombia from my beginnings in the 1980s to the present. I highlight the positive influence of outstanding professors and colleagues in medical training and the evolution of the specialty over the years. However, despite the remarkable technological and pharmacological advances, I assume a critical position in the face of the current situation of Pneumology in the country, pointing out problems such as depersonalized care, the absence of control and supervision in pulmonary function laboratories, as well as the lack of an adequate reading, which allows a clinical correlation, in many radiological and pathology reports. In the same way, he showed the need for greater inclusion and cooperation between pulmonologists and the Colombian Association of Pneumology and Thoracic Surgery, a review of continuing medical education processes and a more transparent and ethical relationship with the pharmaceutical industry.
