We present the case of a 24-year-old patient with sudden and rapidly progressive dyspnea, and a history of silicone injection in the gluteal region 24 hours before hospital admission. Physical examination showed reduced respiratory sounds and tachypnea. Arterial blood gases showed impaired oxygenation. Computed tomography and bronchoalveolar lavage were consistent with silicone pulmonary embolism. This condition has a high risk of death, due to severely altered oxygenation and perfusion. The use of silicone for cosmetic purposes is an illegal practice in the majority of cases, and is life-threatening. The evolution of this case was favorable.
