Parathyroid adenomas are the leading cause of primary hyperparathyroidism. These tumors may be found in atypical sites, such as the mediastinum, and their sizes can vary. During the post-operative period, the bones of some of these patients can have high calcium requirements. This condition, called the hungry bone syndrome, can cause severe crises of hypocalcemia. We report a case of surgical excision of a mediastinal adenoma with subsequent refractory hypocalcemia.
