Cryptogenic organizing pneumonia is an infrequent form of idiopathic interstitial pneumonia, with diverse and nonspecific subacute symptomatology, this entity should be suspect in patients who simulate bacterial pneumonia without improvement to antibiotic treatment, paraclinical findings are variable. The treatment of this pathology varies according to severity.  Case report of an 84-year-old woman with a history of hypertension and dyslipidemia, with six months of respiratory and abdominal symptoms, negative infection studies, immunologic findings suggestive of diffuse interstitial pulmonary disease, organizing pneumonia type. Started treatment with corticosteroids improving the symptoms, however, presented relapse after suspending the treatment. The low frequency of this pathology makes it susceptible to being underdiagnosed, expanding the knowledge in medical literature will allow determining possible causality and therapeutic protocols.
