I am pleased to present this issue of the Colombian Journal of Pneumology in which interesting developments are found. We have modified the cover of the magazine's presentation in which a more dynamic and modern scheme was adopted, in order to give a more relevant space to the images and anticipate the content of the same; In addition, in conjunction with the new National Board of Directors, we made the decision to align ourselves with the global trend of environmental conservation and the national initiative of "zero paper" in the institutions, in such a way that we publish our magazine only in online format .With this measure we reduce the use of renewable resources as valuable as paper, we have more freedom in the handling, design, quantity and quality of images, and we intend to reach many more users with each copy. In this issue, the journal has contributions from pneumology, radiology, thorax surgery, pediatric pulmonology, interventional pneumology and pathology, among others, from Colombia and Latin America; A systematic review of pulmonary rehabilitation is presented in which the results of this intervention are explored in the specific group of patients who have overlapping syndrome of chronic obstructive pulmonary disease (COPD) and asthma. Includes an original article that explores the contribution of early weaning as a risk factor for very severe pneumonia in the infant; In this study, more than 50% of the population had pneumonia and early weaning, suggesting a relationship between pneumonia and the presentation of very severe pneumonia in children, especially among children older than 2 months and younger than 5 years. It is concluded that breastfeeding continues to be a protective factor against several pathologies, among them, respiratory diseases.We present a review article regarding treatment with long-acting inhalers, which are a therapeutic tool for the treatment of patients with broncho-obstructive diseases such as COPD and bronchial asthma. There is also another article that explores the implications and infectious safety profile of biological therapy with omalizumab and mepolizumab in severe refractory asthma, in order to raise awareness of the risks related to infections when using these molecules and generate some recommendations for prevention and management in the Latin American context. In addition, an extensive review of the typical and atypical radiological manifestations of pulmonary sarcoidosis is included, which have a broad spectrum from the typical lymph node involvement (bilateral hilar adenomegalies and right paratracheal adenomas), parenchymal (perilymphatic nodules, bilateral parahiliary opacities, and changes due to fibrosis). ), to atypical manifestations of lymph node involvement (unilateral, asymmetric adenomegalies), parenchymal (alveolar sarcoidosis and lung masses), airway disease, pleural and others (mycetomas).This number also includes several interesting clinical cases such as the development of histoplasmosis in an immunocompetent individual, the enucleation of an esophageal leiomyoma assisted by robotic surgery, the diagnosis and management of a mucoepidermoid tumor of the trachea and the clinical case of a patient with idiopathic pleuroparenchymal fibroelastosis; in addition to the sections of pathological glossary, images in bronchoscopy and information for patients that, in this opportunity, is related to cystic fibrosis.
