        Cryptogenic organizing pneumonia is an inflammatory disease that involves the distal bronchioles, alveolar ducts, and peribronchial alveoli, producing a restrictive pulmonary pattern.

        We present the case of a 33-year-old man with a background of delayed psychomotor development who presented to the emergency department with a clinical picture suggesting lower respiratory infection. The patient was hospitalized and received antibiotics, without clinical improvement. Tomographic findings and poor response to treatment were taken into consideration for performing bronchoscopy with bronchoalveolar lavage and transbronchial biopsy, whose report was conclusive for cryptogenic organizing pneumonia.

        This clinical case shows a patient with a histopathologically confirmed diagnosis and its possible etiological association.
