The SDRA in the adult is an entity characterized by a diffused predicament of the epithelial and endothelial surtaces of the lung, mediated by a large amount of cytokines, which can result in respiratory failure. This results as a consequence of a process of fibroproliferation originated from the activated miofibroblasts of the interstice, that which can rea ch the alveolar space thanks to the continuity solution in the basal membrane, producing a matrix that colors far fibronectin and collagen III.
The progression toward a state offibroprolíferation is characterized functionally by failure of the Peep to improve the oxygenation, by a low distensibílity and by an augment in vascular resistance; clinically it could resemble nosocomial pneumonia. An increase in the neutrophil count from the bronchial alveolar lavage characterizes the early onset of SDRA, a count which should stay elevated in the first ar second week following the event, it is associated with a poor prognosis. In regards to the treatment, preliminary studies suggest that steroids can intervene in the process of fibroproliferation, staying in accordance with the prognosis.
