We inform the case of a 34 year-old man, without previous breathing il/ness who consulted to urgencies for fracture of lukewarm and fíbula. lt presented breathing inadequacy in the moment of their evaluation. 

The x-rey and the thorax TAC showed infiltrated lung diffuse. Microbiological tests that were carried out was negative. The broncho-alveolar lavage (LBA) demonstrated consistent discoveries with foamy macrophages. After the treatment with steroids, the patient showed favorable evolution, from the radiological clinical and blood gases point of view.
