Prevalencia de síndrome metabólico y obesidad en pacientes con síndrome de apnea hipopnea del sueño (SAHOS) en el Hospital Universitario San Ignacio

Prevalence of metabolic syndrome and obesity in patients with sleep apnea­hypopnea syndrome (OSAHS) at Hospital Universitario San Ignacio

Resumen

Introducción: existe una clara relación entre el síndrome de apnea obstructiva del sueño (SAHOS) y el sobrepeso, los cuales a su vez están asociados con aumento del riesgo cardiovascular. En Colombia se dispone de pocos datos sobre la frecuencia del SAHOS, así como de la frecuencia de su asociación con dichos factores. Se describe la prevalencia de obesidad y de síndrome metabólico en los pacientes con síndrome de apnea hipopnea del sueño en el Hospital Univesitario San Ignacio, en Bogotá.

Métodos: se describe la frecuencia de obesidad (índice de masa corporal mayor de 30) en pacientes con SAHOS, perímetro abdominal, valores de colesterol total, HDL, triglicéridos, LDL, glucemia preprandial y TSH, y las diferencias entre aquellos con y sin SAHOS y aquellos con y sin síndrome metabólico.

Resultados: del total de sujetos, 215 tuvieron diagnóstico de SAHOS (66%), 69 no lo tuvieron (21,4%) y 37 tuvieron un resultado de polisomnograma no válido. Se encontró alta prevalencia de síndrome metabólico en el subgrupo de pacienetes analizados. De los pacientes con síndrome metabólico la mayoria tenían SAHOS lo cual no fue estadísticamente significativo (p=0, 155), posiblemente por ser una pequeña muestra de toda la población. No se encontró relación la escala de somnolencia de Epworth y la severidad del SAHOS por el indice de apnea-hipopnea (IAH) (r=0,13).

Conclusiones: al igual que lo reportado en la literatura, en este estudio se encontró alta prevalencia de obesidad y SAHOS en la población estudiada. Se halló alta coexistencia del síndrome metabólico, SAHOS e hipertensión arterial, con lo cual se corrobora que estos pacientes constituyen un grupo de alto riesgo cardiovascular, en quiénes debe hacerse intervención temprana.
